If you’re preparing for plastic surgery, you’re probably focused on the procedure itself — and maybe a little nervous about recovery. What many patients don’t realize is that recovery actually starts before surgery. One of the simplest, most powerful tools we use to support healing and reduce surgical stress for patients having plastic surgery under general anesthesia (asleep) is a pre-surgery carbohydrate drink.

What Is a Pre-Surgery Carbohydrate Drink?

These are specially formulated clear beverages containing complex carbohydrates — different from sugary sports drinks or juices. They’re designed to be easily absorbed, provide energy, and help reduce some of the common side effects associated with fasting before anesthesia.

Why They Matter

For years, patients were told to avoid all food and drink after midnight before surgery. While this reduced the risk of aspiration (stomach contents entering the lungs), it also left patients dehydrated, nauseous, and low on energy.

Research now supports a better approach. Pre-surgery carbohydrate drinks have been shown to:

  • Reduce post-operative nausea and vomiting
  • Stabilize blood sugar, especially during longer surgeries
  • Lessen hunger and thirst before surgery
  • Reduce the body’s stress response to surgery
  • Support better pain control and early mobility
  • Potentially shorten hospital stays

Instead of starting surgery in a depleted state, patients go in with better hydration, stable energy, and improved comfort.

How I Use Them in My Practice

I tailor the timing and quantity of carbohydrate drinks based on the type of procedure:

  • For larger surgeries (like tummy tucks or mommy makeovers):
    • One drink the night before
    • Another drink 2–3 hours before surgery
  • For smaller surgeries (like breast augmentations or lifts):
    • One drink the night before
    • Morning drink is often omitted if an LMA (laryngeal mask airway) is planned, per anesthesia preference

Alternatives and Special Circumstances

Not every patient can tolerate the standard, medical-grade carbohydrate drinks. Some may:

  • Have allergies to ingredients (like corn-based starches)
  • Follow a vegan diet
  • Forget to order them in time (though I provide them to my patients at pre-op visits to avoid this)

In these cases, I recommend Gatorade or Powerade. While not ideal — they lack the full 50g of complex carbs — they are still better than going into surgery completely fasted.

  • Chilling the drink helps with palatability
  • You don’t have to sip slowly — if the taste is too sweet, drink it quickly
  • These alternatives still offer hydration and energy, even if they aren’t part of a formal enhanced recovery protocol

Importantly, these drinks have been well studied and are now part of many ERAS (Enhanced Recovery After Surgery) protocols worldwide. They have also been used safely in diabetic patients, with proper blood sugar monitoring.

What Patients Should Know

  • Only clear, carb-based, medical-grade drinks should be used — not protein shakes, smoothies, or anything with fat or fiber
  • Timing is key — follow exact instructions for when to drink them
  • These drinks are a tool to enhance recovery, not just a trend
  • They do not replace good nutrition or hydration in the days leading up to and after surgery — they’re a final boost, not the only strategy

Recovery starts before your surgery ever begins. Pre-surgery carbohydrate drinks are a low-risk, high-reward tool to improve comfort, reduce complications, and support a smoother healing process. By personalizing the plan to your surgery type — and offering alternatives for those who need them — we can make your surgical experience safer, more comfortable, and better supported.
